NGHIỆM THU ĐỀ TÀI KHCN CẤP THÀNH PHỐ “NGHIÊN CỨU HIỆU QUẢ CỦA PHẪU THUẬT MILES NỘI SOI CẮT TẦNG SINH MÔN TƯ THẾ NẰM SẤP TRONG ĐIỀU TRỊ UNG THƯ TRỰC TRÀNG THẤP” (English version below)

Từ năm 2010, phẫu thuật Miles nội soi cắt tầng sinh môn tư thế nằm sấp đã được Torbjorn Holm và Philip Quirke (bệnh viện Saint Mark, Anh quốc) thực hiện và ghi nhận hiệu quả đáng kể về mặt ung thư học, giảm tỷ lệ tái phát và tăng tỷ lệ sống còn của bệnh nhân. Kể từ đó, phẫu thuật này đã được thực hiện ở nhiều trung tâm trên thế giới. Vì thế, nhóm nghiên cứu gồm các giảng viên tại trường Đại học Y Dược Cần Thơ qua quá trình học tập trực tiếp tại bệnh viện Saint Mark đã tiến hành thực hiện phẫu thuật này nhằm điều trị bệnh lý ung thư trực tràng thấp tại bệnh viện Trường Đại học Y Dược Cần Thơ.

Trong thời gian từ tháng 12 năm 2018 đến tháng 06 năm 2024, có 45 bệnh nhân ung thư trực tràng thấp đã được phẫu thuật tại bệnh viện trường ĐHYD Cần Thơ. Kết quả ghi nhận sau phẫu thuật hơn 3 năm, hơn 85% bệnh nhân vẫn duy trì sức khoẻ ổn định. Tái phát ung thư chỉ có 6,6% và tỷ lệ tử vong rất thấp, khoảng 4%.

Những kết quả này cho thấy rằng phẫu thuật Miles nội soi tư thế nằm sấp là một phẫu thuật an toàn, mang lại hiệu quả điều trị cho bệnh nhân ung thư trực tràng thấp, góp phần giúp bệnh nhân tiếp cận điều trị tiên tiến ngay tại địa phương, giảm chi phí đi xa điều trị,

giảm gánh nặng cho gia đình và xã hội.

Thay mặt Hội đồng nghiệm thu, TS. Ngô Anh Tín, chủ tịch hội đồng đã đánh giá cao ý nghĩa của đề tài, đồng thời đề nghị tiếp tục mở rộng triển khai trong thực tiễn nhằm nâng cao sức khoẻ người dân.

ACCEPTANCE OF THE MUNICIPAL-LEVEL SCIENTIFIC AND TECHNOLOGICAL RESEARCH PROJECT “EVALUATION OF THE EFFICACY OF LAPAROSCOPIC ABDOMINOPERINEAL RESECTION (MILES PROCEDURE) IN THE PRONE JACKKNIFE POSITION FOR THE MANAGEMENT OF LOW RECTAL CANCER”

With the overarching aim of improving postoperative recovery and quality of life in patients with low rectal cancer in the Mekong Delta region, Can Tho University of Medicine and Pharmacy Hospital conducted the research project entitled “Evaluation of the Efficacy of Laparoscopic Abdominoperineal Resection (Miles Procedure) in the Prone Jackknife Position for the Management of Low Rectal Cancer,” co-directed by Nguyen Van Tuan, MD, PhD, and Assoc. Prof. Pham Van Nang, MD, PhD. On September 24, 2025, after five years of clinical implementation and data analysis, the project was officially reviewed and approved by the Department of Science and Technology of Can Tho City. The Scientific Evaluation Council, composed of senior colorectal surgeons, oncologists, and medical scientists, unanimously endorsed the study for its scientific validity, methodological rigor, and clinical relevance.

Since its first description in 2010 by Torbjorn Holm and Philip Quirke at St. Mark’s Hospital, London (United Kingdom), the laparoscopic abdominoperineal resection (APR) in the prone jackknife position has been recognized as a major advancement in the surgical management of low rectal cancer, offering superior oncologic outcomes, including better circumferential resection margins, reduced local recurrence rates, and improved long-term survival compared with the traditional lithotomy approach. Building upon this international experience, the surgical team at Can Tho University of Medicine and Pharmacy Hospital, after receiving direct clinical training at St. Mark’s Hospital, successfully introduced and standardized this minimally invasive technique for patients with low rectal cancer in Can Tho University of Medicine and Pharmacy Hospital.

Between December 2018 and June 2024, 45 patients with low rectal cancer underwent laparoscopic abdominoperineal resection in the prone jackknife position at Can Tho University of Medicine and Pharmacy Hospital. After a median follow-up of more than three years, over 85% of patients maintained stable health, the local recurrence rate was 6.6%, and the disease-related mortality rate was approximately 4%, demonstrating favorable long-term outcomes and excellent postoperative recovery.

These results confirm that laparoscopic abdominoperineal resection in the prone jackknife position is a safe, feasible, and oncologically sound surgical technique for low rectal cancer, enabling patients to access advanced cancer treatment locally. This approach contributes to reducing treatment-related expenses, limiting patient referrals to central hospitals, and alleviating the socioeconomic burden on both patients and the community.

On behalf of the Scientific Evaluation Council, Ngo Anh Tin, MD, PhD, Chairman of the Council, commended the project’s scientific significance and practical value, and recommended its wider clinical implementation to further enhance public health outcomes throughout the region.
